Mail or drop off all paperwork(at Union Elementary School- Attn: Diadel Ortiz) and payments(make check payable to: Community Connections- on the Memo-write child's full-name). Must receive all paperwork by Monday,August 24,2015.

In order to enroll a child in any of our programs, we require the following:
1) A completed Registration Form
2) Copy of Immunization Record
3) A completed Activity Form for the current CC session with Calendar (when available starting in Mid-September)
4) A signed Parent Contract (back page of the CC Parent Handbook)
5) A signed Behavioral Contract (back page of the CC Parent Handbook)
6) Payment in advance for the first week (we do have weekly & Bi-weekly payment plans please speak with the Program Director for more details)
All forms are available on the CC website or in the main office at Union Elementary School.
